Rehabilitation Service Coordinator -

Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.

 

The opportunity:

 We are currently looking to fill one Full-Time 2-year contract Rehabilitation Service Coordinator position at Heritage East Supportive Housing Program.

 

Responsibilities:

·       Effectively manages and approves program resources, human and financial needs to meet program goals and objectives.

·       Recruits, employs, trains, supervises and evaluates support service staff, within a pre-determined budget in accordance with the approved program plans.

·       Coordinates the assessment, development and implementation of rehabilitation plans with assigned staff.

·       Monitors and evaluates operational effectiveness, and implements changes required for improvement.

·       Participates as a member of CSS administration team in planning, problem resolution, quality improvement and review of service delivery performance.

·       Acts as a resource to staff and families in the provision of long term support and service coordination for complex or catastrophic cases to enable adults with a brain injury to remain in community.

·       Participates on community committees where appropriate.

·       Marketing of services for both public and privately funded services.

·       Acts as a staff resource to support activities of Community Support Supervisor

·       On call duties on a rotational basis

·       May include responsibilities amongst other BI programs as needed


Qualifications and experience:

·       Bachelor’s degree in Human Services program or equivalent preferred

·       2 – 5 years experience in the Social Services/Health Sector, coordinating services for individuals with ABI.

·       Concrete knowledge of the effects of an ABI

·       Community based experience working with individuals with an ABI, physical disabilities or special needs

·       Demonstrated experience supervising or leading teams

·       Working knowledge of third party funders and corresponding legislation.

·       Highly organized and excellent time management skills

·       Demonstrated ability to supervise and train employees to include organizing, prioritizing and scheduling work assignments   

·       Problem solving and conflict resolution skills

·       Skill in budget preparation and fiscal management

·       Knowledge of human relations

·       Knowledge of behavior management principles

·       Knowledge of Health & Safety legislation

·       General understanding of computer hardware and software

·       Strong ability to assess and determine effective compensatory strategies for individuals with brain injurie
