Our Community Support Services (CSS) team provides essential services that enable our clients to thrive. Whether their disability is physical, the result of brain injury, or aging, our more than 2,000 team members assist clients in their homes and across communities, to live as independently as possible.
This Opportunity
We are currently looking to fill a Full-Time Community Support Leader position with the Jason House program.
Key Responsibilities:

Provide leadership in the efficient and safe delivery of Community Support Services (CSS) to consumers with disabilities

Lead a team of attendants assisting with non-medical activities of daily living
-
Support the Community Support Supervisor in developing schedules in accordance with each consumer’s plan of service

Assist in renewing and managing consumer service agreements

Facilitate orientation, goal setting, problem-solving, and service planning with consumers

Deliver orientation and on-the-job training to new attendants, and provide ongoing coaching and leadership

Participate in quality improvement initiatives

Communicate with professionalism and diplomacy when interacting with consumers, staff, and external agencies

Manage change positively and supportively

Handle service delivery issues and conflict resolution

Participate in the On-Call/After-Hours rotation
Qualifications:
-
High School diploma or equivalent
-
Current Standard First Aid and CPR certification
-
Experience working with individuals with physical disabilities
-
Understanding of Independent Living or Consumer-based care
-
Knowledge and skill in lifting, transferring, and bowel/bladder routines
-
Completion of Physical Demands Self-Analysis required upon hire
-
Strong interpersonal and communication skills

Flexibility to work various regularly scheduled shifts

Valid driver’s license and/or reliable transportation
